- When YOU are involved in your care, studies show that care is safer, more efficient and cost-effective

Anne, this is a great reminder about the importance of advocacy no matter who you are or where you are in the health system. Thanks for talking about the need for people to have the conversation and complete their advance directive. And you are right, Five Wishes is a great tool for anyone to easily use. What I would remind people is that they do not need a lawyer in order to complete their advance directive, in fact, it is much more cost effective to talk to your healthcare provider and do the documents yourself. Physicians can now be compensated for working with you on your advance directive and it will be easier to make changes to your directive if you do not need to go back to a lawyer to do so when you review the document in the future. Also, you are so right that completing an advance directive and having the conversation when one is well is the ideal time. Thanks for addressing these important issues!!

Thank you for this valuable information on advocacy. Health care should begin with Caring. After reviewing HCAP scores recently it is interesting to note the scores for discharge information provided were consistently low throughout the country. So if the national average is low most hospitals are falling at or below the average. This is another reason why we all need advocates and case managers.
